Changing your contact details

To update your phone numbers or email address:

• fill out a change of contact details form
• phone or visit the surgery from 8am to 6:30pm

If you have moved to a new address, you will need to be in our catchment area to stay registered with our surgery.

• Check you are in our our catchment area. Check the boundaries of GP surgeries in North Ceredigion here

To change your name, gender or title please complete this form, attaching documentation of your name change, for example a copy of your marriage licence, name change declaration, or deed poll.

You can return the form;

• by post
• by returning it to the surgery (surgery post box located next to the main door)
• via email, to contact.w92025@wales.nhs.uk
