Physiotherapy Information (Welsh)

 

To self refer simply fill out the front and back pages of the form. Once you have completed this, send the referral to one of the addresses on the bottom of the form.

Physiotherapy Self Referral Form

The physiotherapy department says "We aim to see urgent referrals within two weeks and routine referrals within 14 weeks of receipt of the form. If you are placed on the urgent list, a letter will be sent to you immediately asking for you to call the department to make a convenient appointment. If you are placed in a routine waiting list, you will be sent a letter that will ask you to phone and make an appointment once you are near the top of that list. If we have no contact from you within a month of the letter having been sent, you will be discharged. A further letter will then be sent to you to confirm that you have been discharged."

 

REFERRED TO OUT- PATIENTS IN HYWEL DDA? 

If you have been referred by your GP to a hospital or clinic urgently you should hear from them in 2 weeks.

If you have been referred to an out patient department in Ceredigion, Carmarthenshire or Pembrokeshire you can call a central number (the contact centre) to find out more information about your appointment.

If you change your contact details you need to inform us. We will then send your address/contact changes to Bronglais Hospital so that they can also update the information that they hold for you.
