Welcome to Ystwyth Medical Group

Merger of Llanilar Health Centre and Ystwyth Medical Group
 communication to registered patients

As you may be aware earlier this year Dr Deanna Evans became a single-handed GP at Llanilar Health Centre.  This means that Dr Evans is solely responsible for the running of the practice services and the business, and this is becoming increasingly unsustainable.

In order to ensure that general medical services can be provided to patients at Llanilar Health Centre, Dr Evans has opened discussions with the GP Partners at Ystwyth Medical Group in Aberystwyth.   After many months of discussion both Practices have agreed that the best way forward is for us to merge to form a new single Practice, so that we will continue to provide care to patients at Llanilar Health Centre and Ystwyth Medical Group.  This merger is scheduled to happen from 1st April 2024 and the practices are busy planning for this.

Dr Evans will be joining Dr Davies, Dr Grahl and Dr Ellaban in partnership and the GPs will be bringing together the Practice staff that currently work in both Practices.  We anticipate that by working together to consolidate services we can continue to attract high quality clinicians to our team.

What does this mean for me?

If you are currently registered with either Llanilar Health Centre or Ystwyth Medical Group, then you will continue to be a patient at the newly formed partnership.  You do not need to do anything.

Where will I be able to access care?

As a patient of the merged practice, you will be able to access services at Llanilar Health Centre or Ystwyth Medical Group, depending on the availability of appointments.  So you can continue to use the Surgery that is local to you, or the other if that’s more convenient if you travel for work for example.

What will the opening times be?

Ystwyth Medical Group will continue to be open 8am – 6.30pm Monday to Friday.  Llanilar Health Centre will remain open with revised opening hours and will have clinicians on-site everyday Monday to Friday.  The Dispensary will remain in Llanilar and will be open daily.  We will be carrying out an exercise over the coming weeks to look at the peak times when the Dispensary is used to make sure this important service remains available for our patients.

If I’m an Ystwyth Medical Group patient, can I collect my medication from the Dispensary in Llanilar?

Dispensary services are available for patients who live more than one mile as-the-crow-flies from a Community Pharmacy and are registered with a Dispensing GP Practice.

Where can I get more information?

The planning for the practice merger in the spring is underway and we will share more detail on our websites as plans progress, and through the surgeries.

The Practices are jointly organising a public engagement drop-in event in the coming weeks in Llanilar and this will be an opportunity for patients registered at either Practice to meet members of the new combined Practice team, along with the opportunity to ask any questions you may have about how services will be provided by the newly merged Practice.  All patients and local stakeholders are welcome to call in to the event, and more details will be made available when the date has been confirmed.

Our patients are incredibly important to us, and we hope that you will continue to offer the same level of support to us as you have done historically as we move forward in our new partnership.

In the meantime, ahead of the merger in the spring, please continue to use services as normal in the practice you are registered at.

Thank you

Uno Canolfan Iechyd Llanilar a Grŵp Meddygol Ystwyth
neges i gleifion cofrestredig

Fel y gwyddoch efallai yn gynharach eleni, daeth Dr Deanna Evans yn feddyg teulu ar ei ben ei hun yng Nghanolfan Iechyd Llanilar. Mae hyn yn golygu mai Dr Evans yn unig sy’n gyfrifol am redeg gwasanaethau’r practis a’r busnes, ac mae hyn yn dod yn fwy anghynaladwy.

Er mwyn sicrhau y gellir darparu gwasanaethau meddygol cyffredinol i gleifion yng Nghanolfan Iechyd Llanilar, mae Dr Evans wedi agor trafodaethau gyda Phartneriaid Meddygon Teulu yng Ngrŵp Meddygol Ystwyth yn Aberystwyth. Ar ôl misoedd lawer o drafod, mae'r ddwy Feddygfa wedi cytuno mai'r ffordd orau ymlaen yw i ni uno i ffurfio Practis sengl newydd, fel y byddwn yn parhau i ddarparu gofal i gleifion yng Nghanolfan Iechyd Llanilar a Grŵp Meddygol Ystwyth. Disgwylir i'r uno hwn ddigwydd o 1 Ebrill 2024 ac mae'r practisau'n brysur yn cynllunio ar gyfer hyn.

Bydd Dr Evans yn ymuno â Dr Davies, Dr Grahl a Dr Ellaban mewn partneriaeth a bydd y Meddygon Teulu yn dod â’r staff Practis sy’n gweithio yn y ddau bractis ar hyn o bryd ynghyd. Rydym yn rhagweld, trwy gydweithio i atgyfnerthu gwasanaethau, y gallwn barhau i ddenu clinigwyr o ansawdd uchel i'n tîm.

Beth mae hyn yn ei olygu i mi?
Os ydych wedi cofrestru ar hyn o bryd gyda naill ai Canolfan Iechyd Llanilar neu Grŵp Meddygol Ystwyth, yna byddwch yn parhau i fod yn glaf yn y bartneriaeth newydd. Nid oes angen i chi wneud unrhyw beth.

Ble byddaf yn gallu cael mynediad at ofal?
Fel claf yn y feddygfa unedig, byddwch yn gallu cael mynediad at wasanaethau yng Nghanolfan Iechyd Llanilar neu Grŵp Meddygol Ystwyth, yn dibynnu ar argaeledd apwyntiadau. Felly gallwch barhau i ddefnyddio’r Feddygfa sy’n lleol i chi, neu’r llall os yw hynny’n fwy cyfleus os ydych yn teithio i’r gwaith er enghraifft.

Beth fydd yr oriau agor?

Bydd Grŵp Meddygol Ystwyth yn parhau i fod ar agor rhwng 8am a 6.30pm o ddydd Llun i ddydd Gwener. Bydd Canolfan Iechyd Llanilar yn parhau ar agor gydag oriau agor diwygiedig a bydd clinigwyr ar y safle bob dydd o ddydd Llun i ddydd Gwener. Bydd y Fferyllfa yn aros yn Llanilar ac ar agor yn ddyddiol. Byddwn yn cynnal ymarfer dros yr wythnosau nesaf i edrych ar yr amseroedd brig pan ddefnyddir y Fferyllfa i wneud yn siŵr bod y gwasanaeth pwysig hwn yn parhau i fod ar gael i'n cleifion.

Os wyf yn glaf Grŵp Meddygol Ystwyth, a allaf gasglu fy meddyginiaeth o’r Fferyllfa yn Llanilar?

Mae gwasanaethau fferyllfa ar gael i gleifion sy'n byw mwy na milltir o Fferyllfa Gymunedol ac sydd wedi'u cofrestru gyda Phractis Meddyg Teulu Cyflenwi.

Ble gallaf gael rhagor o wybodaeth?

Mae’r cynllunio ar gyfer yr uno practisau yn y gwanwyn ar y gweill a byddwn yn rhannu mwy o fanylion ar ein gwefannau wrth i’r cynlluniau fynd rhagddynt, a thrwy’r cymorthfeydd.

Mae’r Practisau’n cyd-drefnu digwyddiad galw heibio ymgysylltu â’r cyhoedd yn Llanilar yn yr wythnosau nesaf a bydd hwn yn gyfle i gleifion sydd wedi cofrestru yn y naill Feddygfa neu’r llall gwrdd ag aelodau o dîm cyfun newydd y Practis, ynghyd â’r cyfle i ofyn unrhyw gwestiynau a allai fod gennych a cael gwybod sut y bydd gwasanaethau'n cael eu darparu gan y Practis sydd newydd ei uno. Mae croeso i bob claf a rhanddeiliad lleol alw i mewn i’r digwyddiad, a bydd mwy o fanylion ar gael pan fydd y dyddiad wedi’i gadarnhau.

Mae ein cleifion yn hynod o bwysig i ni, a gobeithiwn y byddwch yn parhau i gynnig yr un lefel o gefnogaeth i ni ag yr ydych wedi ei wneud yn hanesyddol wrth i ni symud ymlaen yn ein partneriaeth newydd.

Yn y cyfamser, cyn yr uno yn y gwanwyn, a fyddech cystal â pharhau i ddefnyddio gwasanaethau fel arfer yn y practis yr ydych wedi cofrestru ynddo.

Here is a short introduction to the practice with some introductions to staff members

We welcome people of all ages including students to our family friendly medical practice, where we work as a team to provide you with the best possible service.

We are located on the outskirts of Aberystwyth town, near to the Welsh Government offices, with ample car parking and a convenient bus stop.

With patients' needs at the heart of everything we do, our website has been designed to make it easy for you to gain instant access to the information you need. As well as specific practice details such as opening hours and how to register, you’ll find a wealth of useful pages covering a wide range of health issues along with links to other relevant medical organisations.

Get Well, Keep Well

Of course we’re not just here for when you are unwell. Our team of healthcare professionals and back-up staff offer a number of clinics and services to promote good health and wellbeing whatever your medical condition.

Access for wheelchairs is well provided for, with ramp access and automatic doors to the waiting room. However if you need assistance please ask a member of our staff. We have a wheelchair available for use.

The reception area has an induction loop system for patients with hearing aids.

Several of our staff are Welsh speaking.

Zero Tolerance Policy

The Practice takes it very seriously if a member of staff or one of the doctors or nursing team is treated in an abusive or violent way.

The Practice supports the government's 'Zero Tolerance' campaign for Health Service Staff. This states that GPs and their staff have a right to care for others without fear of being attacked or abused. To successfully provide these services a mutual respect between all the staff and patients has to be in place. All our staff aim to be polite, helpful, and sensitive to all patients’ individual needs and circumstances. They would respectfully remind patients that very often staff could be confronted with a multitude of varying and sometimes difficult tasks and situations, all at the same time.  The staff understand that ill patients do not always act in a reasonable manner and will take this into consideration when trying to deal with a misunderstanding or complaint.

However, aggressive behaviour, be it violent or abusive, will not be tolerated and may result in you being removed from the Practice list and, in extreme cases, the Police being contacted.

In order for the practice to maintain good relations with their patients the practice would like to ask all its patients to read and take note of the occasional types of behaviour that would be found unacceptable:

  • Using bad language or swearing at practice staff
    • Any physical violence towards any member of the Primary Health Care Team or other patients, such as pushing or shoving
    • Verbal abuse towards the staff in any form including verbally insulting the staff
    • Racial abuse and sexual harassment will not be tolerated within this practice
    • Persistent or unrealistic demands that cause stress to staff will not be accepted. Requests will be met wherever possible and explanations given when they cannot
    • Causing damage/stealing from the Practice's premises, staff or patients
    • Obtaining drugs and/or medical services fraudulently

We ask you to treat your GPs and their staff courteously at all times.

Removal from the practice list

A good patient-doctor relationship, based on mutual respect and trust, is the cornerstone of good patient care. The removal of patients from our list is an exceptional and rare event and is a last resort in an impaired patient-practice relationship. When trust has irretrievably broken down, it is in the patient’s interest, just as much as that of the practice, that they should find a new practice. An exception to this is on immediate removal on the grounds of violence e.g. when the Police are involved.

Removing other members of the household

In rare cases, however, because of the possible need to visit patients at home it may be necessary to terminate responsibility for other members of the family or the entire household. The prospect of visiting patients where a relative who is no longer a patient of the practice by virtue of their unacceptable behaviour resides, or being regularly confronted by the removed patient, may make it too difficult for the practice to continue to look after the whole family. This is particularly likely where the patient has been removed because of violence or threatening behaviour and keeping the other family members could put doctors or their staff at risk.
