How do I access my medical records via the NHS Wales App?
The NHS Wales app is still in it's testing phase, and this means that some parts of it are not yet working fully. To be ready for access to parts of your record as they become available, please complete the form below (writing that you want access to your records via the app only) and return it to the practice, via email to contact.w92025@wales.nhs.uk or the postbox. You will be able to see parts of your record but will not be able to see your full notes. If you want to have a copy of your full notes, this is currently only available on paper form by completing the form below.
