To ensure that your registration is a processed promptly, please make sure that all the relevant paperwork is filled in correctly and the Patient Contract is signed

Once signed please bring to the surgery or send to along with proof of address.

Failure to provide these documents will cause a delay in processing your registration.

Please leave at least 7 working days for a new registration to be processed. – This may take longer due to the COVID pandemic.

If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Eligibility can be quickly confirmed from your address so please could provide proof by way of a recent bank statement, utility bill in your own name (mobile phone bills are not accepted), tenancy agreement or council tax statement.  We may also require one form of photographic ID.

You will need to complete a registration form (GMS1), a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor and also sign the patient contract. These forms will need to be completed and returned to us as soon as possible so that we can complete your registration.  If you take regular medication, please include your repeat prescription form with your application to register.

Medical treatment is available from the date of registration. Please contact reception for further information.

If you wish to register with the practice please click on the link below to open the form/print the form. Please ensure that you complete all the details including your previous address and Previous GP, failure to do this may result in your registration being held up. When you have completed all of the details, print it out and bring it in with you on your first visit to the practice. When you visit the surgery for the first time please report to reception to sign the registration form so that we can confirm that all of your details are correct. Also by signing the form it ensures your health notes can be requested from your previous practice.

Once the registration form has been filled in and printed, please complete the 'new patient questionnaire' and read and sign the 'patient contract' by clicking on the links below. Print them off and fill them in, bringing them to the surgery together with your 'registration form' and a form of photographic ID e.g. passport and proof of address e.g. utility bill - without these documents we will not be able to process your registration.

Alternatively you may scan all of the documentation and return via email ensuring that the documents have your original signature.
