Please complete the registration form as normal.
- Please ensure you complete the date you first came to UK. You need a valid visa (work, student) and to have paid the ‘Immigration Health Surcharge’ (IHS). You can get a share code to prove this at ‘View and prove your immigration status - GOV.UK (www.gov.uk)’
For registration from Ukraine, please contact the practice. We have registration packs available in Ukrainian and Russian.
